In compliance with the Federal Transparency in Coverage Rule, Richmed Hospice makes available its Machine-Readable Files (MRFs) that include detailed pricing information for items and services covered under our health plans and network arrangements.

These files are published to help patients, families, and the public understand healthcare costs and coverage in a clear and accessible way — even if the data itself is in a technical format.

What Is a Machine-Readable File (MRF)?

A Machine-Readable File is a digital data file — typically formatted in JSON, XML, or CSV — that contains structured pricing information which computers can process automatically.
It is not designed for general consumer use but is made available to support price transparency, regulatory compliance, and third-party data analysis.

Machine-readable files are primarily intended for:

  • Researchers and data analysts,
  • Insurance carriers and payers,
  • Regulatory agencies, and
  • Third-party application developers creating consumer price-comparison tools.

Purpose of the Transparency in Coverage Rule

The Transparency in Coverage (TiC) Rule, issued by the U.S. Department of Health and Human Services (HHS), the Department of Labor (DOL), and the Department of the Treasury, requires health plans and healthcare providers to disclose detailed pricing information for covered items and services.

The rule aims to:

  • Help patients make informed healthcare decisions.
  • Encourage fair competition in healthcare pricing.
  • Promote accountability among insurers and providers.

At Richmed Hospice Services, we fully support this effort and provide the following information as required by law.

Types of Files Published

In accordance with federal guidance, the following types of Machine-Readable Files are made publicly accessible:

  1. In-Network Rate Files
    Contain negotiated rates for covered items and services between Richmed Hospice Services (and its contracted health plans) and in-network providers.
  2. Allowed Amount Files
    Show the historical out-of-network allowed amounts and billed charges for covered items and services, aggregated to protect patient privacy.
  3. Prescription Drug File (if applicable)
    Lists the negotiated rates for prescription drugs between health plans and pharmacy benefit managers (PBMs).
    (Please note: this requirement is currently under review and implementation timelines may vary.)

These files are not intended for patients to estimate their individual costs.
For patient-specific cost estimates or benefits information, please contact your health plan or speak directly with our billing department.

Accessing the Files

The machine-readable files for Richmed Hospice Services and our affiliated plans are available through secure external hosting platforms maintained by our insurance carriers and third-party administrators.

Because the data files are large and complex, they are best accessed using a computer equipped with specialized data-analysis tools.

How Often Are the Files Updated?

  • Files are updated monthly to ensure the most current information is available.
  • Each update reflects any new rates, contract changes, or plan adjustments that may affect pricing data.
  • Historical versions of the files may be archived for audit or compliance reference.

Understanding What’s in the Files

While the data is primarily for technical and compliance use, here’s a brief explanation of what’s included:

Data Field Description
Plan Name / ID Identifies the specific insurance plan or coverage type.
Provider Name / NPI Lists healthcare providers, facilities, or suppliers under contract.
Service Code (CPT / HCPCS) Identifies each covered item or service.
Negotiated Rate Displays the contracted rate between the plan and the provider.
Billing Code Description A plain-language description of the item or service.
Effective & Expiration Dates The dates during which the rates are valid.

This information helps regulators, employers, and data professionals evaluate cost transparency across the healthcare system.
